Local Anesthesia
Since administering sedatives orally, surgeons use a mixture of Lidocaine (Xylocaine) and Bupivicaine, a longer-acting local anesthetic (Marcaine). To reduce the sensation of injections, high frequency vibration is added to the skin when the anesthetic is administered (this is referred to as vibratory anesthesia). To numb the entire scalp, the anesthetic is only applied around the perimeter of the scalp (called a ring block). The rest of the hair restoration surgery is painless until the scalp is numb. If more anesthesia is required, it will be given before the first drug wears off, normally about 5-6 hours after the operation.
Donor Tissue
To maximize yield and minimize scarring, the donor tissue is removed from the scalp in a single thin strip during Follicular Unit Transplantation (FUT) (Single-strip harvesting). The donor area is deliberately selected to be in a region of the scalp with the most hair density and long-term stability. The hair in the area to be removed is trimmed short, to a length of about 1-mm, so that when the patient leaves the office, the uncut hair will fully cover the donor area. When the donor tissue is removed from the scalp, it is subsequently put in a solution. This closely resembles the body’s own fluids and increases the chances of the grafts surviving outside the body. Depending on the case, the donor region is closed with either absorbable sutures (that do not need to be removed) or staples in Follicular Unit Transplantation.
Stereo-Microscopic Dissection
Specific follicular units (each comprising 1 to 4 hairs) are meticulously dissected from the donor strip in their naturally occurring groups under strict stereo-microscopic control in Follicular Unit Transplantation. Extra tissue is removed from the grafts, and the skin between the units is discarded. After that, the follicular unit grafts are sorted into groups of one, two, three, or four hairs. Creating the Recipient Sites
The hair transplant surgeon establishes recipient sites with a fine instrument identical to hypodermic needles. The recipient sites for the majority of procedures are lateral (perpendicular slits). Most of the cosmetic look of the transplant is determined by the recipient locations, which decide the angle at which the new hair grows as well as the distribution and density of the grafts. The construction of a recipient site is an important aspect of hair restoration surgery that necessitates extensive experience, surgical expertise, and a keen aesthetic sense.
Graft Placement
The surgical team will begin inserting the follicular unit grafts into these pre-made sites once all of the recipient sites have been completed. To achieve maximum fullness, one-hair follicular units are used in the frontal hair line to give a soft, natural appearance, and 3- and 4-hair grafts are used in the central forelock region. The most time-consuming aspect of hair loss surgery is the placement. Graft placement is a specific operation, and you will be asked to hold your head relatively still during this time. You can pass the time by watching movies, TV, or sleeping, and you can take breaks as needed to use the toilet, stretch, or eat.
At the End of the Procedure
After all of the grafts have been mounted, the location of each one will be double-checked. The post-operative directions will be explained, and you will be given a printed copy to take home. The doctor’s cell phone number will be included. To cover the donor region, a tennis bandage is wrapped around the head, and a baseball cap or bandana is worn home. The transplanted region does not require any bandages or dressings.
